That is correct. I bought the car new from the dealer. My occupation was one in which I moved every 3 or so years - thus the registration in multiple states.

No, the car has never been used by a smoker. The hole in the seat was from a seat heater malfunction in 1996. The seat heater was repaired but owner did not 'patch' the small hole.
